Job Title: Registered Nurse Care Coordinator - Ambulatory

As a Registered Nurse Care Coordinator - Ambulatory with the Commonwealth of Virginia, you'll play a pivotal role in providing care coordination services to patients across the state. This dynamic position offers a wide range of opportunities for growth and development within our dynamic outpatient clinics.

Responsibilities:

  • Provide facilitative care that promotes health equity across the continuum and among interdisciplinary teams to improve population health, patient experience, and cost reduction.
  • Collaborate and advise patients, families, and caregivers in their healthcare decisions, respecting their culture and values.
  • Develop action plans and guidance to initiate patient-centered care planning and application of the nursing process.
  • Support medication management and other duties as assigned.

Requirements:

  • Education: Bachelor of Science in Nursing from an accredited nursing program.
  • Experience: 1 year of registered nursing experience required, 3 years of related and relevant registered nursing experience strongly preferred.
  • License/Certification: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification is required.
