We are looking for a highly skilled Medicaid Schools Transition Specialist to join our team at the State of New Hampshire. The ideal candidate will have experience in healthcare, education, or a related field and possess strong project management skills.

The selected individual will be responsible for overseeing the Medicaid School-Based Services (Medicaid to Schools) grant, ensuring that the Department of Health and Human Services meets all program objectives and grant deliverables on schedule.

This role involves collaborating with the procured Medicaid to Schools vendor, facilitating the transition of Local Education Agencies (LEAs) to the Certified Public Expenditure (CPE) model, and serving as the main point of contact for the Department's MTS vendor.

The successful candidate will develop and maintain project plans, proactively manage issues, and communicate on project progress. They will apply quality improvement strategies and coordinate with internal and external partners to meet deliverable deadlines.

The estimated salary for this position is $31,330 - $42,600 per year.
