Job Summary

We are seeking a compassionate and detail-oriented Hospice Administrative Assistant to join our team at Vertex Hospice Care Inc. in Santa Clarita, CA.

The ideal candidate will have a minimum of 1 year of experience in hospice or home health care, with proficiency in Axxess Home Health Software. They will be responsible for coordinating patient care and services, monitoring re-certification and re-authorization processes, and ensuring the appropriate use of medications and durable medical equipment.

The successful candidate will possess excellent communication and organizational skills, with the ability to work effectively in a fast-paced office environment. They will also be fluent in English and Spanish, with the ability to communicate with patients, families, and healthcare professionals.

Responsibilities:

  • Coordinate patient care and services, including those of the team, social worker, home health aide, and physician
  • Monitor re-certification and re-authorization processes
  • Ensure the appropriate use of medications and durable medical equipment
  • Educate patients, families, and caregivers on hospice care and services
  • Participate in routine chart reviews and attend in-services and educational conferences

Qualifications:

  • LVN/RN license (non-licensed clinicians/medical staff encouraged to apply with previous hospice experience)
  • Minimum 1 year of experience in hospice or home health care
  • Proficiency in Axxess Home Health Software
  • Excellent communication and organizational skills
  • Fluent in English and Spanish
