Job Overview

AIDS Healthcare Foundation is seeking a dedicated and compassionate Healthcare Navigator to join our team. As a key member of our Access and Adherence Program, you will play a vital role in ensuring that patients enrolled in the AIDS Drug Assistance Program (ADAP) have access to comprehensive healthcare coverage.

Key Responsibilities

As a Healthcare Navigator, your primary focus will be on conducting proactive outreach to patients who are eligible for ADAP benefits. You will work closely with our benefits department to ensure that patients receive the necessary screenings and documentation required to qualify for funding. Your expertise in navigating the complex private insurance industry will be invaluable in helping patients navigate through the system and achieve viral suppression.

In addition to enrolling patients into medical funding, you will also be responsible for identifying any medical or social barriers that may prevent them from achieving their treatment goals. This will involve collaborating with internal and external medical and social resources to provide personalized support and guidance.
