Job Summary

We are seeking a skilled Patient Care Navigator to join our team at Houston Methodist Hospital. As a key member of our healthcare team, you will be responsible for facilitating patient flow and bed utilization, ensuring that patients receive the highest quality care in a timely manner.

Key Responsibilities:
  • Collaborate with clinical staff and interprofessional health care teams to optimize patient placement and resource allocation.
  • Communicate effectively with patients, families, physicians, and other stakeholders to ensure seamless transitions throughout the hospital stay.
  • Evaluate patient needs and identify opportunities for improvement in patient care and department operations.
  • Develop and implement process improvements to enhance patient satisfaction, safety, and outcomes.
Requirements:
  • Bachelor's degree in Nursing or related field required; Bachelor's degree preferred.
  • Two years of experience as a registered nurse in an acute care setting.
  • Bed placement experience highly preferred.
Benefits:
  • $83,500 - $123,000 per year, based on experience.
