About the Role

This is an exciting opportunity for a skilled oncology nurse to join our team as a Cancer Care Navigator RN. As a member of our interdisciplinary team, you will play a vital role in ensuring seamless care transitions, educating patients and families about their treatment options, and advocating for their needs throughout their journey.

Responsibilities
  • Collaborate with healthcare professionals to develop personalized care plans that meet the unique needs of each patient.
  • Educate patients and families about disease-specific information, treatment options, and self-care strategies.
  • Foster relationships with referring physicians to ensure effective communication and coordination of care.
  • Maintain accurate records and documentation of patient interactions and progress.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
  • Bachelor's degree in Nursing (BSN) or higher required; Master's degree in Nursing (MSN) or related field preferred.
  • Current licensure as a Registered Nurse (RN) in the State of Texas.
  • At least three years of experience in oncology nursing or a related specialty.
  • Excellent communication, teaching, and problem-solving skills.
