We are currently seeking a highly skilled Medical Care Navigator to join our team in Houston, TX. As a Medical Care Navigator, you will play a critical role in ensuring that our patients receive high-quality care and support throughout their journey with us.

About the Role

The Medical Care Navigator will be responsible for coordinating care and providing ongoing nursing assessments of patient and family needs. This includes working closely with interdisciplinary teams to develop individualized plans of care and recommending revisions as necessary.

Key Responsibilities

  • Conduct medical home visits on established patients, performing hands-on exams at each visit
  • Conduct urgent/acute visits on established patients with the goal of keeping them out of the hospital and receiving the best medical care at home
  • Provide ongoing assessments of the impact of life-limiting and/or chronic illness(es) on patients' physical, functional, psychosocial, and environmental needs
  • Assess caregiver burnout and provide community resources when appropriate
  • Implement individualized plans of care and recommend revisions as necessary
  • Perform procedures, including wound care and dressing changes, phlebotomy, and injections (B12, vaccines)
  • Conduct advanced care discussions with patients and families and document their wishes in the electronic medical record
  • Consult and educate patients/families and other caregivers regarding disease processes, pain and symptom management, end-of-life care, and processes for dealing with issues of ethical concern
  • Initiate preventive and rehabilitative nursing procedures as needed
  • Ensure continual assessment of patient and family needs from admission to Care Coach Connect
  • Connect throughout the course of care
  • Provide ongoing evaluation of patient and family/caregiver response to care and recommend alterations to the plan when necessary
  • Attend interdisciplinary meetings and serve as a crucial and vocal member of the team
  • Solve problems by gathering and/or reviewing facts and selecting the best solution from identified alternatives

Requirements

  • Registered Nurse with a current license to practice in the state of Texas
  • Current BLS certification or must obtain certification within 30 days of employment
  • 3+ years of experience in a physician's office, clinical, hospice, or hospital setting
  • Demonstrated excellent nursing skills
  • Proven excellent verbal and written communication skills
  • Ability to interact productively with individuals and multidisciplinary teams
  • Proficient computer skills, including Microsoft Word, Excel, Access, and Outlook
  • Excellent organizational and prioritization skills
  • Valid driver's license within the state of work
  • Access to reliable transportation for daily travel to various locations as assigned

Estimated Salary: $70,000 - $90,000 per year, depending on experience
