Physician Assistant or Nurse Practitioner

University of Minnesota Physicians is seeking a highly skilled Certified Nurse Practitioner or Physician Assistant to join our Physical Medicine and Rehabilitation (PM&R) team at the M Health Fairview Acute Rehabilitation Center (ARC) at the University of Minnesota Medical Center.
Key Responsibilities- Deliver exceptional care to patients with complex medical conditions in a rehabilitation setting.
- Collaborate with experienced APPs and academic PM&R physicians to develop and implement individualized plans of care.
- Consult on hospital inpatients to evaluate appropriateness for next level care, including acute rehabilitation settings.
- Perform daily rounds, admissions, discharges, and consults, and complete necessary documentation.
- Participate in multi-professional teamwork to ensure provision of high-quality and efficient patient care.
- Facilitate continuity of care across the continuum of services.
- Educate patients and families regarding disease processes, interventions, discharge transitions, and postoperative and post-procedural care.
- Actively participate in staff education, research, process improvement initiatives, and administrative activities.
- Graduate of an accredited Nurse Practitioner training program or a graduate of an accredited Physician Assistant program.
- Current MN RN and NP license through the Minnesota Board of Nursing or a current MN PA license through the MN Board of Medical Practice.
- Certified through ANCC/AANP/ACNP or eligible for ANCC/AANP/ACNP national exam or through the NCCPA and meet threshold criteria/qualifications for Credentialing and Privileges.
- Excellent written and verbal communication skills.
- Exceptional collaboration abilities.
- Understanding of customer care and importance of detail-oriented and self-motivated work.
- Prior experience in Physiatry or a similar area of specialty.
- One or more years of experience within an inpatient hospital setting.
- Experience with medically complex patients and patients with rehab needs.
