Job Summary

We are seeking a highly skilled and dedicated Clinical Pharmacist to join our team at Ochsner Health. As a Clinical Pharmacist, you will play a vital role in providing exceptional patient care and contributing to the development of our pharmacy services.

Key Responsibilities
  • Provide pharmacokinetic consultation and evaluate drug therapy for assigned patients.
  • Monitor and evaluate medication therapy for appropriateness based on patient health status, institutional criteria, and cost-efficient measures.
  • Develop and implement medication management plans to optimize patient outcomes.
  • Collaborate with healthcare providers to educate patients and families on medication use and management.
  • Participate in the development of pharmacy-related documents, such as order sets and formularies.
  • Contribute to the education and training of pharmacy residents and students.
Requirements
  • Doctor of Pharmacy (PharmD) degree or equivalent.
  • Current pharmacy license in the state of practice.
  • Completion of a PGY1 Pharmacy Practice Residency or 3 years of related specialty experience.
  • Strong leadership and teaching skills.
  • Effective verbal and written communication skills.
Preferred Qualifications
  • Completion of a PGY2 Pharmacy Residency.
  • Doctor of Pharmacy (PhD) degree.
