Job Title: Ambulatory Clinical Pharmacist

Ochsner Health is seeking a highly skilled and motivated Ambulatory Clinical Pharmacist to join our team. As an Ambulatory Clinical Pharmacist, you will play a critical role in providing high-quality patient care and contributing to the development of our ambulatory care services.

Responsibilities:
  • Provide medication therapy management and counseling to patients in ambulatory care settings.
  • Conduct medication reviews and develop individualized medication plans to optimize patient outcomes.
  • Collaborate with healthcare teams to ensure seamless transitions of care and optimal patient outcomes.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
  • Stay up-to-date with the latest clinical guidelines and research to provide evidence-based care.
Requirements:
  • Doctor of Pharmacy (PharmD) degree from an accredited institution.
  • Current licensure as a pharmacist in the state of practice.
  • Experience in ambulatory care pharmacy practice or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
Preferred Qualifications:
  • Board certification in pharmacotherapy or a related field.
  • Experience with electronic health records and other healthcare technology.
  • Knowledge of clinical research and study design.
