Responsibilities- Provide comprehensive medication management services to assigned patients, including medication reconciliation, monitoring, and education.
- Collaborate with healthcare providers to optimize patient care and outcomes.
- Develop and implement clinical pharmacy services, including medication therapy management and disease state management programs.
- Provide education and training to healthcare providers, patients, and caregivers on medication use and management.
- Participate in quality improvement initiatives and contribute to the development of clinical pharmacy policies and procedures.
- Doctor of Pharmacy (PharmD) degree or equivalent.
- Current licensure as a pharmacist in the state of practice.
- Completion of a PGY1 Pharmacy Practice Residency or equivalent experience.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Completion of a PGY2 Specialty Pharmacy Residency or equivalent experience.
- Prior experience in an ambulatory care setting.
- Board certification in pharmacotherapy or a related field.
