Ambulatory Clinical Pharmacist

Ochsner Health is seeking a highly skilled Ambulatory Clinical Pharmacist to join our team. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and medication management services to our pediatric patients.
Responsibilities- Provide pharmacokinetic consultation and obtain laboratory data to evaluate drug selection and dosing.
- Evaluate patient response to drug therapy and document interventions.
- Exhibit good patient outcomes by conducting outcome studies and evaluations.
- Assist in the development of pathways, best practices, and guidelines for assigned populations.
- Educate staff, students, patients, and healthcare providers on medication management and patient care.
- Mentor residents, students, and new staff to meet departmental goals.
- Provide pharmacy expert content for order sets, benefit design, formularies, and other pharmacy-related documents.
- Current Pharmacy license in the state of practice.
- Completion of a PGY1 Pharmacy Practice Residency (3 years related specialty experience accepted in lieu of PGY1 Residency).
- Preferred: Doctor of Pharmacy (PharmD) and Completion of a PGY2 Pharmacy Residency.
The incumbent works with patients who have known or suspected communicable diseases and may enter isolation rooms. The incumbent has an occupational risk for exposure to all communicable diseases. There is potential to encounter hazardous medications and hazardous waste.
