About the Role

We are seeking a highly skilled and motivated Ambulatory Clinical Pharmacist to join our team at Ochsner Health System. As a key member of our dermatology team, you will play a critical role in providing exceptional patient care and contributing to the development of our clinical programs.

Responsibilities
  • Provide comprehensive medication management services to patients with dermatological conditions, including medication therapy optimization and patient education.
  • Collaborate with healthcare providers to develop and implement individualized treatment plans that meet the unique needs of each patient.
  • Conduct medication reconciliation and monitoring to ensure safe and effective medication use.
  • Develop and maintain clinical pathways and guidelines for dermatological conditions.
  • Provide education and training to healthcare providers, patients, and caregivers on dermatological conditions and treatment options.
  • Participate in quality improvement initiatives to enhance patient outcomes and optimize clinical practices.
Requirements
  • Doctor of Pharmacy (PharmD) degree or equivalent.
  • Current licensure as a pharmacist in the state of practice.
  • Completion of a PGY1 Pharmacy Practice Residency or equivalent experience.
  • Strong clinical and communication skills, with the ability to work effectively in a fast-paced environment.
  • Ability to maintain confidentiality and handle sensitive patient information.
