Ambulatory Clinical Pharmacist

We are seeking a highly skilled and dedicated Ambulatory Clinical Pharmacist to join our team at Ochsner Health. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and contributing to the development of our clinical programs.
Key Responsibilities- Provide comprehensive medication management services to assigned patients, including medication reconciliation, monitoring, and education.
- Collaborate with healthcare providers to optimize patient care and develop individualized treatment plans.
- Participate in the education and training of pharmacy residents, students, and other healthcare professionals.
- Contribute to the development of clinical pathways, best practices, and guidelines for ambulatory care.
- Stay up-to-date with current federal, state, and local laws, accreditation standards, and regulatory agency requirements.
- Doctor of Pharmacy (PharmD) degree or equivalent.
- Current pharmacy license in the state of practice.
- Completion of a PGY1 Pharmacy Practice Residency or 3 years of related specialty experience.
- Strong leadership, teaching, and communication skills.
- Ability to work effectively in a team environment and adapt to changing priorities.
