We are seeking a dedicated and experienced RN Care Coordinator II with a BSN to provide exceptional care management for pediatric patients. The ideal candidate will possess a current and unrestricted RN license in Pennsylvania and have a minimum of three years of clinical experience working with medically fragile children, along with telephonic case management experience.

Key Responsibilities:
- Assist members in achieving their optimal health through effective care management and coordination services.
- Engage with members, caregivers, and healthcare providers to assess and establish individual health goals.
- Facilitate and coordinate care while ensuring quality and cost-effective resource utilization.
- Conduct comprehensive assessments to identify physical health history, mental health status, and social determinants of health.
- Manage medication reconciliation and adherence support.
- Develop and implement personalized care plans to address barriers to health.
- Schedule follow-up communications and make necessary referrals to ensure continuity of care.
- Document progress towards health goals and collaborate with care teams.
Qualifications:
To be considered for this role, candidates must possess:
- A Bachelor’s Degree in Nursing.
- A current and unrestricted Registered Nurse license in Pennsylvania.
- A minimum of three years of clinical experience with pediatric patients in various care settings.
- Experience in telephonic case management within a managed care environment is preferred.
- Proficiency in Microsoft Office and electronic medical record systems.
- A valid driver’s license and willingness to travel for in-person member visits as required.
