Homeless Services Case Manager

As a Homeless Services Case Manager at the Homeless Prevention Center, you will play a vital role in supporting clients in achieving their housing goals. Your primary responsibility will be to work with a caseload of 12 to 18 client households, providing regular support and guidance to help them overcome housing challenges.
Key Responsibilities:
- Meet with clients to assess their housing needs and develop individualized housing plans.
- Provide training and referrals to clients to support their goals and address barriers in areas such as financial empowerment, life skills, and tenant education.
- Collaborate with various agencies to support clients in obtaining permanent and safe housing.
- Attend required trainings and be available for additional training opportunities as identified by the Program Supervisor or Executive Director.
- Perform administrative duties, including data entry and preparation of reports as assigned.
Requirements:
- Strong communication and interpersonal skills.
- Ability to work effectively with clients from diverse backgrounds.
- Knowledge of housing resources and services in the community.
- Ability to work independently and as part of a team.
