Job Summary

This role is designed to support individuals with disabilities in their daily lives, fostering a sense of independence and inclusion in their communities. As a Community Integration Specialist, you will work closely with clients to develop and implement personalized support plans, promoting their social, communication, and life skills.

Key Responsibilities
  1. Program Implementation:

    1. Assist in the development and implementation of programs and activities that promote socialization, skill development, and community involvement.

  2. Support and Advocacy:

    1. Provide direct support to clients during community activities, ensuring their safety and well-being.

    2. Advocate for clients' rights and access to community resources and services.

  3. Skill Building:

    1. Teach and reinforce social, communication, and life skills to enhance clients' independence and confidence.

    2. Provide training on using public transportation, accessing community resources, and participating in social activities.

  4. Documentation and Reporting:

    1. Maintain accurate records of clients' progress, activities, and any incidents.

    2. Prepare reports and communicate regularly with supervisors and relevant stakeholders.

  5. Training and Development:

    1. Participate in ongoing training to stay current with best practices in disability support and community integration.

    2. Provide training and support to volunteers and other staff members as needed.

Qualifications
  • High school education

  • Previous experience working with individuals with disabilities

  • Strong understanding of disability rights and inclusion principles

  • Excellent communication, interpersonal, and organizational skills

  • Ability to work independently and as part of a team

  • Patience, empathy, and a passion for promoting inclusivity and empowerment

  • Reliable transportation

  • Punctual and flexible

Working Conditions
  • Researching and documenting using MS Office and other platforms as identified

  • This role may involve working evenings, weekends, and holidays to accommodate clients' schedules and community events

  • Physical activity may be required, including assisting clients with mobility and participating in various community activities

  • Travel within the community is a significant part of the job. Must have reliable transportation and auto insurance
