POSITION SUMMARY

The Program Transition Specialist II serves as the primary support to the Program Transitions Manager, delivering oversight and assistance for designated program initiation and conclusion projects.

Key Responsibilities:

  • Collaboration and Support:
    • Assist the Program Transitions Manager and team with the requirements for launching new programs and managing program closures.
    • Help formulate a comprehensive startup strategy for each new program, ensuring effective execution.
    • Coordinate and monitor progress across various departments involved in program rollout, including: Program, Human Resources, Information Technology, Facilities, and Finance.
    • Conduct ongoing assessments of implementation procedures to promote the successful initiation of new programs.
    • Perform additional tasks as assigned.
  • Liaison Duties:
    • Serve as a point of contact between internal teams and external partners.
    • Work with the Start-Up Program Manager to ensure clear communication among program personnel, internal departments, and funding sources.
    • Keep relevant staff updated on critical deadlines, deliverables, and requests from funders.
    • Participate in both internal and external meetings.
  • Data Management:
    • Maintain and organize documentation related to awards, including Requests for Proposals (RFPs), proposals, contracts, and necessary supporting documents.
    • Track startup requirements and ensure compliance with established timelines.
    • Prepare reports and undertake special projects as needed.
  • Program Compliance:
    • Collaborate with program and administrative staff to ensure the fulfillment of grant and contractual obligations during program launches and closures.
    • Conduct regular site evaluations using a checklist to verify adherence to grant requirements, health and safety standards, and documentation protocols.
    • Assist in the continuous review of startup systems to guarantee effective program initiation.
  • Additional Responsibilities:
    • Perform other duties as directed.

Qualifications

REQUIREMENTS:

  • Must successfully pass a fingerprint clearance and background check, including criminal history, personal references, and verification of employment and education.

EDUCATION:

  • Bachelor's Degree in Social Services, Business, or a related field, or a combination of relevant education and experience.

EXPERIENCE:

  • A minimum of 2.5 years of experience in a social services organization, specifically in supporting grant and contract development.
