Clinical Pharmacist: Pharmacotherapy Specialist

We are seeking a highly skilled Clinical Pharmacist to join our Pharmacotherapy team at Kadlec Regional Medical Center. As a Clinical Pharmacist, you will play a critical role in ensuring optimal use of medications to achieve specific outcomes that improve patients' quality of life.
Key Responsibilities- Collaborate with the interdisciplinary team to provide chronic disease state management to patients, including initiation, modification, and discontinuation of medication therapy.
- Monitor medication therapy for side effects, effectiveness, and allergies, and document appropriately within the electronic health record.
- Provide drug information to patients and professional staff, and participate in medication care service activities as deemed necessary by pharmacy leadership.
- Bachelor's Degree: Graduate from an accredited School of Pharmacy with a BS/PharmD degree or equivalent education/experience.
- Washington State Pharmacist License upon hire.
- One (1) year of PGY-1 residency or equivalent experience.
- Two (2) years of PGY-2 AmCare residency or equivalent experience.
- Certification in Ambulatory Care upon hire.
