Key Responsibilities- Provide comprehensive pharmaceutical care to patients, including medication therapy management and patient education.
- Collaborate with the healthcare team to optimize patient outcomes and quality of life.
- Develop and implement individualized medication plans to meet the unique needs of each patient.
- Monitor and evaluate patient responses to medication therapy, making adjustments as needed.
- Stay up-to-date with the latest advances in pharmaceutical care and apply this knowledge to improve patient outcomes.
- PharmD or Bachelor's Degree in Pharmacy
- Washington Pharmacist License
- Basic Life Support-Provider (BLS) certification
- Acute care hospital pharmacy experience preferred
