Home Infusion Nurse

Optum is seeking a skilled Home Infusion Nurse to join our team. As a Home Infusion Nurse, you will provide clinical care and education to patients receiving parenteral medications in the comfort of their own homes. You will work collaboratively with a multidisciplinary team to prepare, deliver, and seek reimbursement for infusion therapy.
Key Responsibilities- Establish and Maintain Positive Relationships: Develop and maintain strong relationships with patients, families, and caregivers to ensure seamless delivery of care.
- Assess Patient Appropriateness: Evaluate patients' suitability for home infusion therapy, considering factors such as safety, caregiver support, and willingness to manage therapy independently.
- Obtain and Document Medical History: Gather and document patients' medical histories, particularly as related to the diagnosis being treated with infusion therapy.
- Complete Clinical Documentation: Thoroughly and timely complete all required clinical documentation, including nursing plans of care, visit notes, and medication profiles.
- Participate in Case Conferences: Collaborate with the multidisciplinary care team to comprehensively plan patient care.
- Provide Skilled Care: Deliver skilled care to patients of varying ages and medical conditions, as outlined in the plan of care and in accordance with physician orders.
- Establish and Secure Vascular Access: Place and maintain vascular access devices, including peripheral IVs, PICCs, and tunneled central venous catheters.
- Administer Medications: Administer medications as prescribed, performing therapy-specific clinical assessments in accordance with company policies and protocols.
- Communicate Patient Response: Communicate observations of patient response to therapy and overall physical assessment/condition with the care team.
- Active RN Licensure: Hold active and unrestricted licensure as a Registered Nurse in the state(s) of practice and others as requested.
- Basic Life Support Certification: Possess current Basic Life Support (BLS) and/or CPR certification, or be willing to obtain prior to starting employment.
- Infusion Therapy Experience: Have 1+ years of experience working as an RN in a clinical care setting that contributes to proficiency in infusion therapy.
- Electronic Medical Record Proficiency: Be proficient with electronic medical record documentation.
- Interdisciplinary Collaboration: Be willing and able to work with an interdisciplinary team of professionals, including pharmacists, technicians, physicians, and registered dietitians.
- Flexibility and Reliability: Be willing and able to work independently in home or alternate-site settings, assume a flexible work schedule, and meet attendance, overtime, after-hours on-call, and other reliability requirements of the position.
