Core Responsibilities:
- Engage in efforts to minimize unnecessary placements and identify relatives for children in care.
- Facilitate connections and resources for families, children, and adolescents.
- Lead interdisciplinary team discussions to promote the success of case plans.
Key Duties:
- Support parents in building networks for their children.
- Identify, inform, and assist relatives for potential foster care placements.
- Provide guidance on placement alternatives and resource accessibility.
- Collaborate with service providers and perform evaluations.
- Work alongside team members and maintain accurate documentation of activities.
Job Requirements:
- Exceptional communication abilities.
- Understanding of child welfare systems and family dynamics.
- Strong analytical and decision-making skills.
- Capacity to prioritize tasks and adapt to evolving situations.
- Proficient in various software applications and platforms.

Qualifications:
