The Southcentral Foundation (SCF) is seeking a highly skilled Registered Nurse to join our Nutaqsiivik Program as a Case Manager. This role is responsible for providing comprehensive nursing services to women and children, promoting quality and cost-effective healthcare outcomes.
Key Responsibilities- Deliver timely and beneficial care to customers, working within established standards for Home Visiting Programs.
- Collaborate with multidisciplinary teams to ensure seamless care coordination.
- Develop and implement individualized care plans to meet the unique needs of each customer.
- Provide education and support to customers and their families on health and wellness topics.
- Participate in quality improvement initiatives to enhance program outcomes.
- Bachelor's Degree in Nursing (BSN) from an accredited School of Nursing, or Associate Degree and enrolled in a Bachelor's program.
- Licensed as a Registered Nurse in the State of Alaska.
- Basic Life Support (BLS) certification required; ACLS, NRP, ATLS, ALSO, and PALS certifications as required for specialty.
- Two (2) years of experience in Maternal/Child Health, Public Health, Home Visiting, or Mental/Behavioral Health Nursing.
- Certification as a Lactation Educator, or obtain within six (6) months of hire.
- Completion of Nurse Family Partnership training within six (6) months of hire.
- Native American or Alaska Native ancestry.
- Experience working with diverse populations, particularly Alaska Native and American Indian communities.
