About the Role

We are seeking a highly skilled Oncology Navigator Coordinator to join our team at Presbyterian Healthcare Services. As a key member of our healthcare team, you will play a vital role in coordinating patient care and providing exceptional patient experiences.

Key Responsibilities
  • Perform visit closure activities to coordinate patient care and ensure seamless transitions.
  • Advanced scheduling for specialty or radiologic procedures, recognizing payor requirements and obtaining prior authorization/certification/referrals as needed.
  • Process daily referral workqueues for authorization, deferring only those that require payor consideration.
  • Monitor in-baskets and reports for overdue results, waitlisted appointments, recalls, and other critical patient information.
  • Provide lay navigation to oncology patients with stable and chronic conditions, coordinating care and services as needed.
  • Collaborate with the nurse navigation team to implement and coordinate plan of care for patients and families, improving health and quality of life outcomes.
  • Develop relationships with cancer physicians and APCs, serving as a liaison between patients and physicians.
  • Assist with preparation of and coordination of the weekly cancer multidisciplinary conference.
Requirements
  • High School Diploma or equivalent required.
  • 2+ years of healthcare experience, with a focus on oncology or patient navigation.
  • BLS certification required at time of hire.
  • Lay Navigator certificate within 6 months of hire.
