Job Title: Community Health Worker

Join our team at Community Health Centers of Burlington as a Community Health Worker, where you will play a vital role in improving patient health and well-being.

Job Summary:

We are seeking a highly motivated and compassionate Community Health Worker to join our team. As a Community Health Worker, you will be embedded into our patient care teams to provide screening, follow-up, and navigation to services for mental health and substance use disorders, as well as social determinants of health (SDOH). Your goal will be to facilitate access to services and improve patient health outcomes.

Key Responsibilities:
  • Provide outreach to patients identified through population health data or care team members.
  • Assist in developing a plan of care, create appropriate referrals, and manage a panel of patients to increase health outcomes and general wellness.
  • Help bridge the gap between diverse patient populations and the healthcare system.
  • Collaborate with the patient's primary care provider, social work, and nursing, as well as other community service providers.
Requirements:
  • High school diploma or equivalent.
  • Previous experience providing social services, translation services, case management services, or other client or customer services.
  • Bi-lingual speaker/writer preferred.
  • Strong written and verbal communication skills.
  • Ability to complete all written paperwork in a timely manner.
Working Conditions:

This position requires sitting and standing, walking, driving, or access to transportation. Occasional evening or Saturday hours may be required.
