Job Summary

COTS is seeking a dedicated and compassionate Outreach Case Manager to join our team. As a key member of our outreach team, you will work directly with individuals and families experiencing homelessness, providing support and guidance to help them achieve stable housing.

Key Responsibilities
  • Build and maintain relationships with clients, addressing their unique needs and challenges.
  • Travel to client locations to provide on-site support and assistance.
  • Help clients navigate the process of securing rental subsidies and other housing assistance programs.
  • Connect individuals with local resources and services to support their transition to stable housing.
  • Develop and implement housing support plans to prevent homelessness and ensure client stability.
  • Collaborate with referral agencies and community partners to ensure seamless service delivery.
  • Provide retention services to clients who have obtained housing, including support with communication with landlords and connection to community resources.
Requirements
  • A minimum of a high school degree or equivalent is required; a bachelor's degree in social work or a related field is preferred.
  • A minimum of one year of prior experience in direct service work is required; two years of experience plus relevant coursework is preferred.
  • Candidates with experience working with individuals experiencing homelessness and crisis intervention are preferred.
  • Ability to work with individuals with mental health, medical, substance abuse, and employment issues is required.
  • Familiarity with Vermont Agency of Human Services regulations and procedures is preferred.
  • Reliability, independent decision-making, and strong written and verbal communication skills are essential.
