Clinical Nurse Navigator

The Clinical Nurse Navigator is a key member of the Central Health team, responsible for providing support for psychiatric services in outpatient settings. This role involves working collaboratively with an interdisciplinary team to intervene therapeutically and support patients with mental illness, substance abuse disorder, personality disorder, substance abuse, or developmental disability.
Key Responsibilities
Assess the physical, social, and emotional needs of patients and families in clinics
Review clinical progress of patients and coordinate communications to referring, consulting, and primary care physicians
Collaborate with the patient care team to optimize clinical outcomes
Participate in daily patient care huddles and weekly clinic staff informational huddles
Perform clinical duties as assigned and competency validated in accordance with Standing Delegation Orders
Conduct transitions of care activities with the patient's medical home care management team or the Central Health case management team as indicated
Requirements
Associate's Degree from an accredited school of nursing required
Bachelor of Science in Nursing (BSN) preferred
~2 years of nursing experience in an ambulatory/outpatient behavioral health setting required
~ Psychiatry nursing experience preferred
~ Background in managing populations with complex medical needs preferred
~ Experience with Epic preferred
Certifications
Certified Care Manager certification preferred
