This navigation position is based out of St. David's Medical Center and focuses on Breast oncology patients.

As an Adult Oncology Nurse Navigator, you will be responsible for:

  • Serving as a patient advocate from diagnosis to end of active treatment.
  • Assessing patients' medical, social, and psychosocial needs/barriers to care and linking them to appropriate resources.
  • Providing appropriate teaching, outreach, and support to patients and families and ensuring the patient is empowered to manage their own healthcare needs.
  • Conducting follow-up conversations with all patients and communicating any concerns, changes, or social needs to the appropriate physician or other care providers.

Requirements

  • Three to five years oncology nursing experience.
  • Experience in program development preferred.
  • Experience as a nurse navigator and/or patient educator in oncology preferred.
  • Experience in leadership and customer service – preferred.

Education

  • Associate's Degree in Nursing required.
  • Bachelor's Degree in Nursing preferred.

Licenses and Certifications

  • Basic Cardiac Life Support (BCLS) certification.
  • Current licensure as a Registered Nurse (RN).
  • Oncology Certified Nurse (OCN) achieved or in progress – preferred.
