About the Role

As a Registered Nurse Case Manager at Crossroads Hospice & Palliative Care, you will play a vital role in delivering high-quality patient care in the comfort of their own homes or care facilities. Your expertise and compassion will be essential in building trusting relationships with patients and their families, ensuring they receive the dignity, respect, and compassion they deserve.

Key Responsibilities
  • Deliver direct patient care, addressing their medical, emotional, and social needs
  • Collaborate with interdisciplinary teams to develop and implement personalized care plans
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
  • Provide education and support to patients and families on disease management, treatment options, and end-of-life care
  • Document patient progress, outcomes, and care plans in a timely and accurate manner
Requirements
  • 6 months minimum prior skilled nursing experience
  • Active, unencumbered state Registered Nurse license
  • Current driver's license and auto liability coverage
  • Interest and ability in providing patient care in homes and facilities
