Key Responsibilities- Deliver direct patient care, addressing their medical, emotional, and social needs
- Collaborate with interdisciplinary teams to develop and implement personalized care plans
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
- Provide education and support to patients and families on disease management, treatment options, and end-of-life care
- Document patient progress, outcomes, and care plans in a timely and accurate manner
- 6 months minimum prior skilled nursing experience
- Active, unencumbered state Registered Nurse license
- Current driver's license and auto liability coverage
- Interest and ability in providing patient care in homes and facilities
