Job Summary:

As a LTSS Service Coordinator-RN Clinician at Elevance Health, you will play a vital role in the LTSS care coordination process. You will work under the direction of an RN, contributing to the identification, evaluation, coordination, and management of members' needs. Your responsibilities will include assisting the RN with telephonic or face-to-face assessments, identifying members for high-risk complications, and obtaining clinical data as directed by the RN. You will also assist the RN in identifying members who would benefit from alternative levels of care or other waiver programs. Your goal will be to provide high-quality care to our members, ensuring their needs are met and their health is improved.

Key Responsibilities:

  • Assist the RN with telephonic or face-to-face assessments to identify, evaluate, coordinate, and manage members' needs.
  • Identify members for high-risk complications and assist the RN in developing a plan to mitigate these risks.
  • Obtain clinical data as directed by the RN to support the care coordination process.
  • Assist the RN in identifying members who would benefit from alternative levels of care or other waiver programs.
  • Participate in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities as directed by the RN.

Requirements:

  • Requires an LPN/LVN, LSW, LCSW, or LMSW or license other than RN in accordance with applicable state law and Nursing Diploma or AS in Nursing or a related field.
  • Minimum of 2 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
  • Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.

Preferred Skills:

  • Proficiency in Microsoft Office (Excel, Word, Outlook, etc.) strongly preferred.
  • Case management experience strongly preferred.
  • Home health experience strongly preferred.
  • Medicaid experience strongly preferred.
  • Discharge planning experience strongly preferred.
  • MA/MS in Health/Nursing preferred.

Work Environment:

This is a field position, requiring travel to worksites and other locations as necessary. The ideal candidate will be able to work independently and as part of a team, with excellent communication and interpersonal skills.
