Job Summary:

UnitedHealth Group is seeking a highly skilled Obstetrical Field RN Care Coordinator to join our team. As a key member of our care coordination team, you will be responsible for managing a panel of members with complex medical and behavioral needs.

Key Responsibilities:

  • Conduct comprehensive needs assessments to identify members' medical, behavioral, and socioeconomic needs.
  • Develop and implement person-centered care plans to address members' needs, including management of chronic health conditions, health promotion, and wellness.
  • Partner with internal care teams, providers, and community resources to implement care plans.
  • Provide education and coaching to support members' self-management of care needs and lifestyle changes.
  • Support proactive discharge planning and manage care transitions following ER visits, inpatient, or Skilled Nursing Facility (SNF) admissions.

Requirements:

  • Current, unrestricted RN license in the state of Tennessee or compact state licensure.
  • Unrestricted driver's license and reliable transportation within the service area.
  • 2+ years of work experience with assigned specialty population.
  • 2+ years of clinical experience in a hospital, acute care, home health/hospice, or case management.
  • Intermediate computer/typing proficiency to enter and retrieve data in electronic clinical records.

Preferred Qualifications:

  • BSN, Master's Degree or Higher in Clinical Field.
  • CCM certification.
