Job DescriptionJob Summary: The Care Coordinator LPN will be responsible for providing patient care services, including monitoring patient health, administering basic care, and collaborating with healthcare professionals to ensure comprehensive care. This role requires strong communication and interpersonal skills to effectively interact with patients, visitors, and healthcare team members.

- Collaborate with healthcare professionals to ensure comprehensive patient care and satisfaction.
- Provide education and support to patients and their families on health-related topics.
We're looking for a dedicated and compassionate Licensed Practical Nurse with a strong commitment to delivering exceptional patient care. To be successful in this role, you should possess the following qualifications:
- Current LPN licensure in the state of practice.
- Basic Life Support (BLS) certification.
- 2+ years of experience in a healthcare setting, preferably as an LPN.
- Strong communication and interpersonal skills.
