Responsibilities

The CSR serves as a liaison between Hospice Home Care Planners, Social Workers, and RN Case Managers. This role requires strong communication skills to respond to patient and customer complaints and grievances.

  1. Respond to Social Work, RN Case Manager, patient, and customer complaints and grievances.
  2. Enter patient referral information into the Allscripts system, including demographics, diagnosis, and plan of care.
  3. Verify new physician license numbers, NPI registration, and check for duplicate entries.
  4. Contact Physicians, Clinics, Hospitals, Nursing Homes, and sub-acute facilities to develop and follow up on pending referrals.
