Jamaica Hospital Medical Center, a 404-bed fully accredited community-based teaching hospital and level I Trauma and Stroke Center, is seeking a full-time experienced Bilingual (Cantonese/Mandarin) Nurse Liaison to join our Care Management Department.

Job Summary

The successful candidate will be responsible for facilitating PCP engagement and follow-up post-hospitalization, assessing care plan development and managing overall clinical needs of patients within the nursing scope of practice, and utilizing EMR data and other sources to perform full medical record reviews and formulate patient health status.

Responsibilities
  • Telephonic Case Management and administration of Transition of Care, Complex Case Management, Co-Managed Care or Home-based services for patients recently discharged from the hospital
  • Facilitating PCP engagement and follow-up post-hospitalization
  • Assessing care plan development and managing overall clinical needs of patients within the nursing scope of practice
  • Utilizing EMR data and other sources to perform full medical record reviews and formulate patient health status
  • Administering disease management education to patients with chronic conditions
  • Effectively collaborating with patients, caregivers, Medical Advisors, PCP, and other specialist community entities to ensure individualized care and completion of care plan goals
Requirements
  • Must be a graduate of an Accredited BSN Program
  • NY State RN License required
  • Bilingual Cantonese/Mandarin Speaking required
  • Must have 2-5 years of inpatient nursing experience
  • Complex Case Management experience preferred
  • Proficiency in assessment and care plan development
  • With proven demonstrated experience with PC and EMR documentation
Estimated Salary: $85,000 - $110,000 per year
