Hospital Liaison Nurse Job Summary:

Northwell Health is seeking a highly skilled Registered Nurse to fill the role of Hospital Liaison Nurse. In this position, you will serve as a liaison between patients, families, and healthcare teams to ensure seamless transitions of care.

Key Responsibilities:
* Serve as a liaison between patients, families, physicians, and healthcare teams
* Conduct clinical reviews to ensure appropriateness of admission and continued length of stay
* Facilitate throughput and coordinate patient transfers
* Ensure compliance with current state, federal, and third-party payer regulations

Requirements:
* Graduate from an accredited School of Nursing
* Bachelor's Degree in Nursing, preferred; currently enrolled in or committed to obtaining BSN within 5 years
* Minimum 2 years relevant experience, required
* Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed

The estimated salary for this position is $98,000 - $120,000 per year, based on location and experience.
