About the Role

As a skilled RN, you will be responsible for assessing patient needs, developing personalized care plans, and coordinating with healthcare teams to ensure seamless care delivery. Your expertise will be invaluable in helping patients achieve optimal health outcomes and improving their overall quality of life.

Key Responsibilities
  • Conduct comprehensive patient assessments to identify care needs and develop tailored care plans
  • Coordinate with healthcare teams to ensure timely and effective care delivery
  • Review patient insurance information and coordinate activities involved in each individual care plan
  • Collaborate with family and professional caregivers to ensure continuity of care
  • Current NYS RN license and registration required
  • Bachelor's degree in nursing preferred; Associate's degree required
  • Minimum of one year acute medical-surgical nursing experience required
  • Certified Home Health Agency (CHHA) experience preferred
  • Bilingual English and Spanish or Mandarin or Cantonese preferred
