Nurse Care Manager

Overview
RiverSpring Health Plans is seeking a compassionate and dedicated Nurse Care Manager to join our team. As a key member of our interdisciplinary team, you will be responsible for providing care coordination, including in-home assessment, planning, facilitation, advocacy, and authorization of covered plan services to meet the member's health needs while promoting quality, cost-effective outcomes.
Responsibilities
The Nurse Care Manager will collaborate with members and/or family members to maintain the most independent living situation possible, ensuring consistent care along the entire healthcare continuum by assessing and closely monitoring member's needs and status. You will be responsible for authorizing covered services and coordinating care regardless of payer.
Qualifications
Must possess a current NYS RN License and registration. Bachelor of Science degree in Nursing required with a minimum three (3) years of RN experience. Clinical expertise in geriatrics preferred; managed long-term care experience preferred. Bilingual skills may be required as determined by operational needs. Must be bilingual Russian, Chinese, or Spanish.
