About the Role:

We are seeking a skilled RN to join our team as a Care Coordinator. In this role, you will be responsible for coordinating and implementing home care plans for patients. Your excellent communication and documentation skills will be essential in ensuring the success of this role.

Responsibilities:

• Review patient insurance information and coordinate activities involved in each individual care plan.
• Share pertinent findings with family and professional caregivers.
• Utilize excellent communication, documentation, and time management capabilities to ensure the delivery of high-quality care.

Qualifications:

• Graduate from an accredited School of Nursing; BSN preferred.
• Minimum of 1 year acute, Med-Surg Nursing experience - required.

Shift:
Monday - Friday, 9:00 am - 5:00 pm

Salary:
$100,000 - $119,000

Location:
Queens, NY, Bronx, NY, Brooklyn, NY

Compensation:
$100,000 - $119,000 per year
