Position Overview

MedStar Health is seeking a qualified Speech Language Pathologist on a per visit basis to enhance our Home Care services. The ideal candidate will possess a valid license in Speech Language Pathology and demonstrate exceptional clinical and interpersonal skills.

The Speech Language Pathologist will conduct evaluations and provide speech-language pathology interventions in compliance with agency protocols, as well as the legal and regulatory frameworks governing speech-language pathology services in the relevant jurisdictions.

Key Responsibilities:
  • Facilitates specialized assessments that are not feasible in a home setting (e.g., audiological evaluations, video fluoroscopy assessments) and ensures proper discharge planning, including referrals to suitable community resources (e.g., outpatient therapy, daycare) that cater to the client's post-discharge needs. Maintains communication with supervisors regarding the delivery of rehabilitation services and patient care.
  • Fosters effective collaboration with other departments and engages in multidisciplinary quality improvement initiatives. Participates in meetings and represents the department in community outreach activities. Pursues professional development through educational programs and affiliations. Maintains ongoing communication with the patient's healthcare team and other relevant parties regarding care and patient status.
  • Conducts assessments of the patient's living environment and recommends adaptive equipment or home modifications to enhance functional abilities and safety. Assists in obtaining items that qualify for reimbursement as durable medical equipment. Provides initial and ongoing assessments to identify the patient's rehabilitation needs. Develops and modifies a comprehensive speech-language pathology treatment plan in collaboration with the patient, their family, and the healthcare team. Evaluations include interviews, observations, and specialized assessments such as dysphagia assessments and language evaluations.
  • Documents medical records in accordance with agency standards and regulatory requirements for reimbursement of speech-language pathology services. Educates patients and caregivers about the injury or condition, functional limitations, and treatment objectives, along with training on care management post-discharge. Delivers speech-language pathology services as per the treatment plan and regulatory guidelines. Supervises home health aides in relevant patient care activities.
Qualifications:
  • Master's degree in Speech-Language Pathology.
  • 1-2 years of clinical experience.
  • Licensed to practice Speech-Language Pathology in the relevant state or hold a Certificate of Competency from ASHA if applicable.
  • CPR certification.
  • Valid State Driver's License in good standing.
