Per Visit Speech Language Pathologist

MedStar Health is seeking a qualified Speech Language Pathologist to provide per visit services within our Home Care division. The ideal candidate will possess a valid license in Speech Language Pathology and demonstrate exceptional clinical and interpersonal skills.
The Speech Language Pathologist will conduct assessments and deliver speech-language therapy in compliance with agency protocols, as well as state and federal regulations governing speech-language pathology services.
Key Responsibilities:
- Coordinate specialized assessments that cannot be performed in a home setting, such as audiological evaluations and video fluoroscopy studies. Ensure proper discharge planning, including referrals to suitable community resources that can support the client post-discharge. Maintain regular communication with supervisors regarding service delivery and patient care issues.
- Foster effective collaborations with various departments and engage in multidisciplinary quality improvement initiatives. Actively participate in meetings and community outreach programs, representing the department and agency. Pursue continuous professional development through educational opportunities and affiliations.
- Conduct comprehensive assessments of the patient's living environment and recommend necessary adaptive equipment or home modifications to enhance functional capabilities and safety. Assist in obtaining durable medical equipment as needed. Develop and adjust a tailored speech-language pathology treatment plan in collaboration with the patient, their family, and the healthcare team. Evaluations will include interviews, observations, and specialized assessments such as dysphagia evaluations and language assessments.
- Document patient records in accordance with agency standards and regulatory requirements for speech-language pathology services. Educate patients and caregivers about their conditions, functional limitations, and treatment objectives, while providing training for post-discharge care management. Deliver speech-language therapy services as outlined in the treatment plan, adhering to professional guidelines and agency standards. Supervise home health aides in relevant patient care activities.
- Master's degree in Speech-Language Pathology.
- 1-2 years of clinical experience.
- Valid license to practice Speech-Language Pathology in the relevant state or a Certificate of Competency from ASHA if applicable.
- Current CPR certification.
- Valid State Driver's License in good standing.
