POSITION SUMMARY

The Community Outreach Coordinator plays a vital role in the marketing and sales efforts of Signature Healthcare at Home. This position is focused on building and nurturing positive relationships with referral sources to achieve targeted admissions and maintain a robust patient census.

KEY RESPONSIBILITIES

  • Identify and develop a network of referral sources, including hospitals, skilled nursing facilities, senior living communities, medical clinics, and healthcare providers to drive admissions.
  • Create and implement marketing strategies and promotional activities to meet established volume goals.
  • Conduct ongoing market analysis to inform a comprehensive marketing plan that aligns with budgetary targets and identifies opportunities for enhanced services.
  • Facilitate effective communication among staff, patients, families, caregivers, and external partners to ensure seamless service delivery.
  • Represent Signature Healthcare with professionalism and integrity to all stakeholders.
  • Assist in the development of annual budget projections for program volume and manage resource allocation effectively.
  • Engage in community outreach and educational initiatives to promote services and enhance visibility.
  • Monitor and shape community perceptions of Signature Healthcare as a premier service provider.
  • Maintain an in-depth understanding of marketing principles, industry regulations, and competitive landscape.
  • Generate referrals from both existing and new customers within the designated territory.
  • Support recruitment and training of new personnel as needed.
  • Participate actively in team meetings and quality improvement initiatives.
  • Assist clients in accessing community resources when services are not provided by Signature Healthcare.
  • Be available for on-call admissions as required.
  • Accept additional responsibilities as assigned by the supervisor.
  • Contribute to the overall growth and success of Signature Healthcare.

QUALIFICATIONS

EDUCATION AND EXPERIENCE:
  • Bachelor's degree in Marketing, Business Administration, or a related field; a master's degree is preferred.
  • A minimum of three to five years of experience in healthcare marketing management, ideally within hospice or home health sectors.
SKILLS:
  • Proficient in managing diverse patient demographics.
  • Strong marketing acumen with the ability to engage tactfully with clients and the community.
  • Solid understanding of business management principles.
  • Excellent communication, negotiation, and public relations skills.
  • Capable of fulfilling essential job functions with or without reasonable accommodation.
  • Demonstrated organizational skills and effective time management.
  • Basic computer proficiency and familiarity with office technology.
  • Valid driver's license and access to a reliable vehicle.
