Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Care Specialist. The successful candidate will be responsible for providing patient-centered care to individuals with terminal illnesses, working closely with an interdisciplinary team to develop and implement individualized care plans.

Key Responsibilities:
  • Conduct initial and ongoing assessments of patients' physical, functional, spiritual, psycho-social, and environmental needs
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Provide education and support to patients and their families on disease management, self-care techniques, and end-of-life care
  • Communicate effectively with the medical director and other clinicians to ensure seamless care coordination
  • Document patient observations, findings, and services provided in a timely and accurate manner
Requirements:
  • Active RN license
  • Valid driver's license, auto insurance, and reliable transportation
  • Strong verbal and written communication skills
  • Ability to complete patient charting on a daily basis
  • Passion for providing high-quality patient care
