Full-Time After Hours On Call Visit Hospice Registered Nurse

Work Schedule:

7 days on and 7 days off, 5:30 PM to 8:30 AM, Monday through Thursday, 5:30 PM Friday and all day Saturday and Sunday, week on and week off.

Key Responsibilities

This role involves providing nursing care outside of standard business hours. The RN will serve as the primary field contact for Triage nursing, responding to incoming calls during shifts. Responsibilities include conducting patient visits and addressing any follow-up needs for patients and caregivers, as well as managing scheduled visits after hours. The nurse will coordinate and direct Hospice and/or Home Health care as necessary, focusing on the holistic needs of patients and families, emphasizing palliative care for hospice patients and rehabilitative care for home health patients.

Patient Care:

Deliver professional nursing care by applying all elements of the nursing process as defined in the state Nurse Practice Act.

Assess and evaluate the patient's condition by identifying patient and family/caregiver needs, participating in the revision of care plans in collaboration with the attending physician or hospice medical director as the patient's status evolves.

Work independently to initiate appropriate preventive, rehabilitative, or palliative nursing procedures. Administer medications and treatments as prescribed in the patient's care plan.

Engage patients and families/caregivers to address needs and achieve related goals.

Provide health care instructions to patients as appropriate based on assessments and care plans.

Assist patients in achieving self-sufficiency or obtaining suitable caregiving and optimal comfort care.

Communication:

Document patient services and status accurately and timely in accordance with professional standards and reimbursement requirements.

Record changes in pain/symptom management as necessary.

Communicate with physicians regarding patient needs and report any changes in patient conditions; obtain necessary orders as required.

Collaborate with on-call hospice providers to adjust the patient's level of care as needed.

Coordinate with community health professionals to implement the care plan.

Educate patients and families/caregivers on self-care techniques as appropriate, providing instructions on medication, diet, and other care as ordered by the physician, while recognizing opportunities for health counseling.

Facilitate continuity of care by working closely with patients, families/caregivers, and other team members.

Support the emotional needs of patients and families/caregivers in cooperation with the hospice interdisciplinary team.

Participate in required staff meetings and interdisciplinary group meetings or case conferences.

Report services provided to the agency clinical leadership team and/or RN Case Manager at the end of each shift.

Qualifications and Skills
  • Graduate of an accredited nursing program.
  • One to two years of recent acute care experience in a healthcare setting.
  • One to two years of recent experience in hospice or home health care.
  • Currently licensed as a Registered Nurse in the relevant state(s).
  • CPR certification required.
  • Effective communication skills, both written and verbal, with all levels of staff.
  • Exceptional interpersonal skills, both over the phone and in person.
  • Strong observation, verbal and written communication skills, problem-solving abilities, and basic math skills; nursing skills as per competency checklist.
  • Self-directed with the ability to work independently.
  • Physical capability to perform nursing duties, including lifting, positioning, and transferring patients, as well as handling supplies and equipment.
  • Must possess a valid driver's license and have access to an insured vehicle.
