Signature Healthcare at Home is seeking a skilled and compassionate Hospice Registered Nurse Case Manager to join our team. As a key member of our interdisciplinary team, you will provide high-quality patient care and coordinate services to meet the needs of patients and their families.
Key Responsibilities
- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
- Coordinate with healthcare providers and community resources to ensure seamless care transitions.
- Collaborate with the interdisciplinary team to develop and implement care plans that meet the unique needs of each patient.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
- Current licensure as a Registered Nurse in the state of practice.
- Minimum of two years of experience in hospice or palliative care nursing.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
