SUMMARY: The role involves delivering social work assistance to young patients dealing with bleeding disorders while overseeing data management and reporting for various research initiatives.

KEY RESPONSIBILITIES:

Social Work Duties:

  • Perform comprehensive psychosocial evaluations of pediatric patients with bleeding disorders, identifying any behavioral, emotional, or psychological issues and their impact on health, care access, coping mechanisms, and support networks.
  • Establish and maintain strong relationships with patients and their families through various communication methods.
  • Offer mental health counseling and therapeutic services to ongoing patients.
  • Collaborate with healthcare professionals to create tailored service plans for patients and families, addressing medical access, financial concerns, and psychosocial support, while referring to relevant internal and community resources.
  • Work alongside clinical teams to ensure coordinated patient care, including prior authorizations and home health services.
  • Document all interactions and completed tasks accurately in the clinic's charting system and relevant databases.
  • Provide individual and family counseling to meet the emotional needs of patients and their families.
  • Evaluate uninsured patients for eligibility for insurance coverage and assist with applications for various assistance programs. Educate patients on Medicare and Medicaid options.
  • Deliver educational resources about bleeding disorders and reinforce information provided by other healthcare team members.
  • Effectively collaborate with project teams to enhance quality and process improvement initiatives.
  • Demonstrate professionalism and courtesy in all interactions with patients, families, and healthcare colleagues.

Data Management Responsibilities:

  • Oversee the development and implementation of processes for managing the CDC Community Counts Project, including:
    • Post-clinic submissions and registry management.
    • Monitoring CDC specimen testing for relevant registries.
  • Ensure access to and training on the American Thrombosis and Hemostasis Network (ATHN) Clinical Manager database.
  • Conduct data queries, generate reports, and perform audits and analyses for the bleeding disorders program.
  • Carry out additional tasks as assigned by management.

QUALIFICATIONS:

  • Master's Degree in Social Work.
  • A minimum of 2 years of experience in clinical social work.
  • Experience working with patients suffering from chronic illnesses.
  • Proficiency in managing patients with both physical and psychological comorbidities.
  • Ability to assist patients facing high levels of distress and coping difficulties.
  • Experience addressing access to care challenges, including those in remote locations.
  • Experience leading support groups for patients and families.
  • Experience in a multidisciplinary healthcare environment.
  • Strong collaborative skills to work effectively with various health professionals in a dynamic setting.
  • Excellent communication skills, both written and verbal.
  • Strong organizational, analytical, and problem-solving abilities.
  • Attention to detail and strong interpersonal skills.
  • Sound judgment and the ability to make timely decisions.
  • Fluency in English and Spanish.
