Pediatric Social Services Specialist

SUMMARY: The role involves delivering social work assistance to young patients facing bleeding disorders while overseeing data management and reporting for various research initiatives.
KEY RESPONSIBILITIES:
Social Work Duties:
- Perform comprehensive psychosocial evaluations of pediatric patients with bleeding disorders, identifying any behavioral, emotional, or psychological issues and their impact on health, care access, coping mechanisms, and support networks.
- Establish and maintain strong relationships with patients and their families through various communication methods including phone, email, and in-person meetings.
- Offer mental health counseling and therapeutic services to patients as needed.
- Collaborate with healthcare professionals to create tailored service plans for patients and families, addressing medical access, financial assistance, and psychosocial support, while referring to appropriate community resources.
- Work alongside clinical teams to ensure comprehensive patient care coordination, including prior authorizations and home health services.
- Document all interactions and services provided in both clinical records and relevant databases.
- Facilitate individual and family counseling sessions to support the emotional well-being of patients and their families.
- Evaluate uninsured patients for potential insurance coverage, assist with applications for financial aid, and educate them on available Medicare and Medicaid options.
- Provide educational resources on bleeding disorders and reinforce information shared by other healthcare team members.
- Engage with project teams to enhance quality and process improvement initiatives.
- Demonstrate professionalism and courtesy when interacting with patients, families, and healthcare colleagues.
Data Management Responsibilities:
- Oversee the implementation of processes for the CDC Community Counts Project, which includes:
- Post-clinic data submissions and registry management.
- Monitoring CDC specimen testing for the Community Counts Registry and Clinical Inhibitors.
- Ensure proficiency in using The American Thrombosis and Hemostasis Network (ATHN) Clinical Manager database.
- Conduct data queries, generate reports, and perform audits and analyses for the bleeding disorders program.
- Carry out additional tasks as assigned by management.
QUALIFICATIONS:
- Master's Degree in Social Work.
- A minimum of 2 years of experience in clinical social work.
- Experience working with patients who have chronic health conditions.
- Proficient in managing patients with both physical and psychological comorbidities.
- Skilled in supporting patients facing high levels of distress and coping difficulties.
- Experience addressing access to care challenges, including transportation issues for remote patients.
- Experience leading support groups for patients and families.
- Ability to collaborate effectively in a multidisciplinary environment.
- Strong written and verbal communication skills.
- Excellent organizational, analytical, and problem-solving abilities.
- Attention to detail and strong interpersonal skills.
- Good judgment and the ability to make timely decisions.
- Fluency in English and Spanish.
