Job Overview

SUMMARY: The role involves delivering social work assistance to young patients facing bleeding disorders while overseeing data management and reporting for various research initiatives.

KEY RESPONSIBILITIES:

Social Work Duties:

  • Perform comprehensive psychosocial evaluations of pediatric patients with bleeding disorders, identifying any associated behavioral, emotional, or psychological challenges and their impact on medical care, support systems, and coping mechanisms.
  • Establish and maintain strong relationships with patients and their families through various communication methods.
  • Offer mental health counseling and therapeutic services to patients as needed.
  • Collaborate with healthcare professionals to create tailored service plans that address medical, financial, and psycho-social needs, facilitating referrals to community resources as necessary.
  • Work alongside clinical staff to ensure seamless coordination of patient care, including authorizations and home health services.
  • Document all interactions and services provided in the clinic's records and relevant databases.
  • Provide counseling to individuals and families to support their emotional well-being.
  • Evaluate uninsured patients for potential insurance coverage and assist with applications for financial aid and public assistance programs.
  • Educate patients about bleeding disorders and promote adherence to treatment plans.
  • Engage with project teams to enhance quality and process improvement initiatives.
  • Exhibit professionalism and empathy when interacting with patients, families, and healthcare colleagues.

Data Management Responsibilities:

  • Oversee the implementation of processes for the CDC Community Counts Project, including:
    • Post-clinic data submissions and registry management.
    • Monitor CDC specimen testing for registries and clinical inhibitors.
  • Maintain proficiency in the American Thrombosis and Hemostasis Network (ATHN) Clinical Manager database.
  • Conduct data queries, generate reports, and perform audits for the bleeding disorders program.
  • Execute additional tasks as assigned by management.

QUALIFICATIONS:

  • Master's Degree in Social Work.
  • A minimum of 2 years of experience in clinical social work.
  • Experience working with patients suffering from chronic illnesses.
  • Proficient in managing patients with both physical and psychological comorbidities.
  • Skilled in supporting patients facing high levels of distress.
  • Experience addressing access to care challenges, including those related to geography and transportation.
  • Background in leading support groups for patients and families.
  • Experience in a multidisciplinary healthcare environment.
  • Ability to collaborate effectively with diverse health professionals in a dynamic setting.
  • Exceptional communication skills, both written and verbal.
  • Strong organizational and analytical capabilities.
  • Detail-oriented with excellent interpersonal skills.
  • Sound judgment and decision-making abilities.
  • Fluency in English and Spanish.
