Job Summary:

The State of Kansas is seeking a highly skilled Social Worker Specialist to join our team at Osawatomie State Hospital. As a Social Worker Specialist - Benefits Coordinator, you will play a critical role in ensuring the seamless transition of patients from hospital care to community-based services. Your expertise in benefits coordination and discharge planning will make a significant impact on the lives of our patients and their families.

Key Responsibilities:

  • Coordinate benefits for patients to ensure a smooth transition from hospital care to community-based services.
  • Work closely with the unit social work specialist to complete and establish benefits for discharge planning.
  • Assist in completing assessments and provide coverage in the social work specialist's absence to ensure discharge planning continues.
  • Develop and implement a robust benefits process to ensure patients have the best possible outcome at discharge.

Requirements:

  • Bachelor's degree in Social Work or a related field.
  • Must possess a license or temporary license to practice Social Work in the State of Kansas.
  • Prior experience working with individuals who have psychiatric diagnoses is preferred.

Working Conditions:

This position may require physical strength to frequently push, pull, twist, bend, squat, walk, stand for extended periods of time, lift and/or restrain residents with or without assistance and respond to aggressive resident behavior with specific, trained techniques. Must be able to observe and respond to residents in distress.
