Client Support Coordinator

Pathways Health and Community Support, LLC is seeking a dedicated Client Support Coordinator to join our team. This role is essential in providing comprehensive support to individuals aged 60 and above who are facing severe mental health challenges and may be experiencing homelessness or are at risk of it.
Key Responsibilities
As a Client Support Coordinator, you will:
- Conduct thorough intake assessments and develop individualized care plans in collaboration with clients.
- Facilitate access to essential community resources and monitor the effectiveness of support services.
- Maintain regular contact with clients through home visits to offer personal support and guidance.
- Document all activities and maintain accurate records in compliance with organizational and governmental standards.
- Build and nurture professional relationships with various agencies and service providers.
- Meet monthly productivity goals while ensuring high-quality customer care.
- Empower clients by identifying their strengths and supporting their journey towards recovery and wellness.
- Contribute to a diverse and inclusive organizational culture.
- Engage in continuous learning to enhance service delivery and maintain industry leadership.
Qualifications
The ideal candidate will possess:
- A Bachelor's degree in human services or a related field.
- At least two years of relevant experience or equivalent experience as a transitional age youth client.
- A valid driver's license and proof of automobile insurance.
- Strong documentation, organizational, and communication skills.
- Intermediate PC skills.
