Kintegra Health is seeking a highly skilled and compassionate Social Worker/Case Manager to join our team. As a key member of our interdisciplinary team, you will provide direct social work case management services to our Senior Total Life Care participants in a home and community-based model of care.
Key Responsibilities- Participate in initial assessments, care planning, and periodic re-assessments of participants' cases, ensuring timely completion of assessments prior to scheduled team meetings.
- Maintain accurate and up-to-date written case management records, including periodic reassessments of program participants.
- Provide individual and family counseling, develop and lead group counseling and activities, and assist participants in accessing community resources.
- Consult with and advise staff members on social, emotional, and cultural factors related to health and medical care, and advocate for participants' needs.
- Encourage effective relationships among staff to promote team building and a cohesive team environment.
- Participate in program and policy development of the Social Work component, and assist with coordination of 24-hour care delivery.
- Document participant changes and communicate updates to team members as needed, and update participants' care plans throughout the reassessment period.
- Collaborate with the interdisciplinary team to promote unity and teamwork, and support Senior TLC's mission, vision, and values.
- Master's Degree in Social Work (MSW) or Licensed Clinical Social Worker (LCSW) preferred.
- Minimum of 4 years of experience in a health and human services field, with 3 years of experience working with a geriatric population.
- LMSW, LISW, or LCMSW licensure, valid NCDL, and vehicle required.
- Individual therapy skills for geriatric participants, analytical thinking, and problem-solving abilities essential.
